Is taking a breath after reciting a verse, surah, or other parts of the prayer obligatory or not?

While reciting in prayer, if I continue to doubt whether I pronounced a word or pronounced it correctly, what is the ruling? Should I repeat the whole sentence continuously or just repeat the doubtful word, with or without continuation?

In the name of Allah, if you have completed the sentence and entered a new one, then doubted a word from the previous sentence, you must ignore the doubt and continue your prayer. However, if you are still in the middle of the sentence or have not yet moved on to the next sentence, you must recite the entire sentence correctly again; simply repeating the doubtful word is not sufficient. Allah knows best.

While reciting a surah in prayer, I forgot what I was reciting and what to continue with, can I start over from the beginning?

In the name of Allah, if you are unsure what to do, you should begin reciting from the start of the surah, and after completing the prayer, you must perform two Sajdahs of forgetfulness. Allah knows best.
